What is a health assessment?

Our health assessment includes a professional report and is a comprehensive professional nursing evaluation of your patients’ personal health risks, including:

Health Needs Assessment

How does our health assessment work?

Discovery

First, our regulated nurses (RN or LPN) will obtain your patients’ basic health information over the phone, including their current medications and vaccinations. Then, we’ll schedule an in-person health assessment visit at their home.

Assessment

Our 2-hour assessment includes reviewing their medical history, family health history, physical assessment, daily living assessment (including physical and mental health), as well as setting personal health goals and supporting their health concerns.

Report

Within 24 hours we’ll provide a written report of their current health status and risks, which can be used to communicate with other healthcare professionals.

Support

With our subscription service, our nurses provide monthly health guidance and mental health support, including monitoring blood pressure, blood glucose levels, accompanying your patients to their appointments, as well as providing mental health counselling services and emotional support for your patients and their caregivers.

Our government approved Alberta Aids to Daily Living (AADL) assessor can provide a free equipment needs assessment and approve government funding for equipment that your patients may need in their home to support their independence for aging in place.

What is a senior's home safety risk assessment?

Our comprehensive 240-point home safety and risk assessment is conducted by our nationally certified Senior Home Safety Specialists™. This 2-hour assessment includes educating your patients, their family and caregivers on home safety and potential risks around their house, as well as providing an extensive report with an action plan to reduce risks related to home safety, fall prevention, financial exploitation, and personal safety.

Discovery

First, our Senior Home Safety Specialists™ will visit your patients’ home and conduct a comprehensive 240-point home safety and risk assessment.

Assessment

Our 2-hour assessment identifies all potential risks to home safety, including how to minimize or prevent falls and injuries, personal safety and financial exploitation.

Report

We also educate your patients, their family and caregivers on how to minimize their home and personal safety risks with an extensive report listing actionable steps.

Support

With our monthly subscription service, Senior Home Safety Specialists™ can provide four hours of ongoing home safety support, including providing resources and guidance to access government funding to support aging in place and if needed, referring your patients to other organizations that can provide support.

Annj Ridsdale-Weddell, MBA, BSCN, RN, GNC(C), SHSS

Co-Founder, RNCare Inc

As a Registered Nurse (RN) for over 30 years, Annj has had a lifelong passion for helping others and a deep-rooted curiosity about what’s important to people. As a result of her passion for helping those in need, Annj co-founded RNCare, a company rooted in the nursing values of safety, compassion and trust.

In 2020, Annj became the first person in Alberta to be certified as a Senior Home Safety Specialist™ by Age Safe® Canada. Annj has also completed a Bachelor of Science in Nursing (BSCN), a Master of Business Administration (MBA), has had a Gerontological Nursing Certification for over 20 years and is an Advisor Member for both Age Safe® Canada and Age Safe® America.

Together, RNCare looks forward to using our knowledge and experience to help the many Albertans who wish to age in place with dignity, in a safe environment and with the highest quality of life possible.

Senior Home Safety Specialist™, RNCare Southern Alberta

Stacy is the Southern Alberta representative for RNCare and a certified Senior Home Safety Specialist™ serving Calgary and surrounding areas. She is the second person to be certified as a Senior Home Safety Specialist™ in Alberta by Age Safe® Canada.

Stacy has lived in Calgary for over 20 years where she raised her family. She worked for the City of Calgary for most of her career, understanding the local needs of people in Calgary. She knows the people and what’s important to them – Calgarians value honesty and care a great deal about each other and their community.

 

I feel so fortunate to have the specialized knowledge to assist the seniors and adults in my life who are aging in place and I am excited to meet many new people in Calgary and neighboring communities who I can help.
